Care Gaps – Any gaps (Medical Conditions) observed in the Care provided to the Patient will be identified and then required help is extended.
This can be two ways
Patients may also come up with a requirement on a particular Care Gap like (Cardiac, Kidneys or Diabetic Conditions)
Providers may identify that this particular Patient is in need of this care for a particular Care Gap.
